29 CFR 1910.22(a)(3): Walking-working surfaces were not maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice: a. On or about March 15, 2017, in front of the back door entrance-Mangrove restaurant kitchen area, employees were exposed to trip and fall hazards when walking along 1/2 inch-1 inch depth floor holes created by two broken 17 inches x 17 inches ceramic floor tiles. b. On or about June 14, 2017, in front of the dishwashing machine-Mangrove restaurant kitchen area, employees were exposed to trip and fall hazards when walking along 1/2 inch depth floor hole created by one broken 17 inches x 17 inches ceramic floor tile. NOTE: Certification of abatement is not required for this citation item as abatement was documented by the compliance safety and health officer during the inspection.
