29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): A.) On or about Thursday, March 9, 2017 at 47-05 Metropolitan Avenue, Ridgewood, NY 11385 (Warehouse) The employer failed to ensure powered industrial truck operators were trained and evaluated prior to forklift operation resulting in an employee being struck by a forklift within the warehouse. ABATEMENT DOCUMENTATION IS REQUIRED PURSUANT TO 29 CFR 1903.19
