29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. (a) On or about 03/09/2017@ approximately 11:30am an employee was hospitalized after receiving 2nd and 3rd degree burns from a back flash while heating the core of a precast tank. The employer did not report the in-patient hospitalization until 3/13/2017 @ approximately 11:15 am.
