29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware shall be provided by the employer for isolating, securing or blocking of machines or equipment from energy sources. a. On or about March 6, 2017, located at Plant 8, employees were not provided with lockable devices for employees to lockout pneumatic ball valves for the installation and testing of a job made pneumatically-rammed seal between a dust collection bin and purge hopper, thereby exposing the employees to the unexpected release of 95-100 psi of stored energy.
