29 CFR 1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures. a) Die Casting Area- On or about April 4, 5, 6, 2017, the employer failed to create energy control procedures that clearly and specifically outlined the requirements for testing machines and equipment, such as Die Casting Machines #5 and #18, to determine and verify the effectiveness of the lockout devices that were applied. No abatement certification or documentation is required for this item.
