29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: a) 3rd Level of 3 Buck Scaffolding Positioned in The Front Center Roof Area of Structure - Worker stepped off the unguarded end of an scaffolding outrigger on to the roof of the structure allegedly to retrieve a tool. When attempting to step back on to the scaffolding from the roof, the worker fell approximately 22 feet to the ground below, on or about 02/28/2017. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
