29 CFR 1926.251(a)(4): The employer did not ensure special custom design grabs, hooks, clamps, or other lifting accessories, for such units as modular panels, refabricated structures and similar materials, were marked to indicate the safe working loads and were proof-tested prior to use to 125 percent of their rated load: On or about February 23, 2017, at Structure 23, prior to using a field modified spreader bar to lift a reel of conductor wire, the employer did not proof test the spreader bar to 125 percent of the load rating.
