29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): On or about March 1, 2017, the employer failed to develop specific documented procedural steps for shutting down, isolating, blocking, securing machines or equipment, placement, removal and transfer of lockout devices or tagout devices, verifying the effectiveness of lockout devices, tagout devices, and other energy control measures to control hazardous energy on the following machines that compose Line 3: a) Kaps-All Packing Systems - Unscrambler/Placement Machine, Model AU-3, S#5826 b) Resina Ball Placement Machine, Model 44-115, S#947237 c) Cap Placement Machine Employees were required to perform servicing tasks, such as unjamming and cleaning, on the individual attachments/equipment without deenergizing the entire line. Equipment serviced individually exposed employees to fractures, lacerations, and amputations. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
