29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following: a) Line 3 Machinery (Plant 2): On or about January 22, 2017, the employer did not ensure fall protection was used while repair related activities were conducted from on top of the machinery near the spin dryer which exposed an employee to falls up to 10-feet to the ground below.
