29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about February 16, 2017, the employer did not develop, document, and utilize a procedure for the control of hazardous energy when employees re-thread plastic sheets at the extrusion lines when a sheet breaks, exposing employees to crush/amputation hazards. No abatement certification or documentation is required for this item.
