29 CFR 1910.28(b)(2)(ii): When any portion of a guardrail system, gate, or chains was removed, and an employee must lean through or over the edge of the access opening to facilitate hoisting, employees were not protected from falling by a personal fall arrest system: (a) Upper woodshop mezzanine; On or about February 20, 2017, personal fall arrest systems were not worn during hoisting operations while the guardrail was removed and employees leaned over an unprotected edge.
