29 CFR 1915.73(d): When employees were exposed to unguarded edges of decks, platforms, flats, or similar flat surfaces, more than 5 feet above a solid surface, the edges were not guarded by adequate guardrails meeting the requirements of 29 CFR 1915.71(j)(1) and (2). (a) The employer did not ensure that fall protection was being used when conducting welding and cutting work on a ship. On or about February 10, 2017, an employee was injured by falling through a hole in the deck that had been previous enlarged.
