29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: (a) Wyvern Site, Track Location OOC 418.1 - On or about January 24, 2017, the employer exposed employees to an engulfment hazard, in that employees working in an unprotected trench, ranging from 6 to 9 feet deep, were not instructed in the recognition and avoidance of unsafe conditions and the regulations applicable to working in and around trenches and excavations.
