29 CFR 1910.140(c)(12): Anchorages used to attach to personal fall protection equipment were not independent of any anchorage used to suspend employees or platforms on which employees work. On or about January 29, 2017, on the rooftop of a four story building where employees were exposed to a seventy-foot fall hazard when cleaning windows using a Sky Genie rope descent system without utilizing an independent anchorage for fall protection.
