29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: a. On or about January 25, 2017, roof area, back side of roof, employees working near two skylights were not protected from falling 12 feet through the (2' x 4') skylights to the floor below by a personal fall arrest system, covers or a guardrail system around each skylight.
