29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): The employer is failing to provide lockout/tagout procedures; the procedures are not written, implemented, and enforced for maintenance of machines, such as but not limited to, printing and bagging machines, and conveyors. Procedures did not include specific procedures on steps to shut down machines, such as use of control panels and/or emergency stops, specific steps on how to isolate and the equipment to use such as disconnect switches and air valves, specific steps on what lockout devices to be used and when to use them, specific steps for testing energy isolation, and how to enforce compliance with lockout/tagout. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citation(s).
