The employer fails to assure effective protection against falls as required by the standard, as demonstrated on January 12, 2017, when employees are working on a 4:12 pitch low slope residential roof and are not protected by any means of personal fall protection, guardrails or safety nets exposing them to falls in excess of 9 feet to the ground below.
