29 CFR 1910.22(d)(1): Walking-working surfaces were inspected, regularly and as necessary, and maintained in a safe condition: (a) The employer did not ensure that the floor surface was clear of rough areas that could cause slips, trips or falls. On or about January 4, 2017, an accident occurred when a cart carrying zinc ingots was caught on the rough floor surface in the manufacturing area.
