OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of electrical arc flash injuries. (a) Location - 417 Main Street, Malden MA: On or about 12/20/2016, the employer failed to ensure that an employee was provided with and used appropriate equipment (S&C Grappier) to safely operate S&C pad mounted switchgear, which exposed the employee to electrical arc flash injuries.
