29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: (a) Inspection of the energy control procedures was not being done in accordance with the standard, in that the auditor was not actually present while the procedure was being done. For example, operators, who were authorized employees, performed lockout as part of Blending Cell Wet Wash procedures (e.g., Blending Cell 5 Organic Wet Wash Procedure). After completion of the lockout procedure steps, the operator called over the supervisor, an authorized employee, to verify that all appropriate parts have been locked out.
