29 CFR 1910.213(r)(4): Suitable guard(s) were not provided to reduce to a minimum the hazard due to the point of operation of the unprotected saw blade: (a) Facility, 116 N. River Avenue, Parker, PA: On or about 12/07/16 a suitable guard was not provided on the CTD miter cut off saw to protect the operator from the rotating blade. The guard on the saw became stuck in the "up and/or unguarded" position and did not provide protection from the rotating blade.
