29 CFR 1926.451(a)(1): Each scaffold and/or scaffold component was not capable of supporting, without failure, its own weight and at least 4 times the maximum intended load applied or transmitted to it: Note: Exceptions are provided in paragraphs (a)(2), (a)(3), (a)(4), (a)(5) and (g) of this section. Location: 9 Norwalk Avenue, Westport, CT 06880 A 2x6 nailed to the side of a building as a scaffold support bracket was not secured properly, dislodged from the building, and was unable to support at least 4 times the maximum intended load of the two employees. On or about 4 December 2016, two workers were working from a carpenter bracket scaffold at a residential construction site in Norwalk, CT. One of the 2x6 support brackets dislodged from the side of the building when the nails holding it in place pulled out from the wall. The 2x12 plank the workers were standing on subsequently and simultaneously shifted, fell a couple of feet, and rested on a lower level. This resulted in both workers losing their balance resulting in both workers falling to the ground 35 feet below. One worker was fatally injured and the other worker sustained severe bodily injuries.
