29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure: a) Job site, 406 Rose Drive, Cranberry Township, PA: On or about December 1, 2016, during roofing activities, the employer did not ensure that appropriate fall protection systems/devices were used where fall hazards of up to approximately 20 feet existed. An employee fell from the roof and was fatally injured.
