29 CFR 1910.147(a)(2)(ii)(B): An employee was required to place a part of his or her body into an area on a machine or piece of equipment where work was actually performed upon the material being processed (point of operation) or where an associated danger zone existed during a machine operating cycle. On or about October 31, 2016 and at times prior thereto, the employer failed to prevent employee(s) from placing themselves over rotating blades during machine adjustments.
