29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section. The employer is failing to protect employees from caught in and amputation hazards. This was most recently documented at the establishment located at 15 SW. 40 Ave in Great Bend KS, in that machine specific energy control producers were not developed, documented or utilized for the following pieces of equipment: a) The SleevCo machine at the F-Line, utilizing electrical, pneumatics and liquids b) The Oden Filling machine at the Cube-Line, utilizing electrical, pneumatics and liquids c) On or about 11/17/16, an employee engaged in production activities at the Bekum Blow Molding machine in the plastics department was exposed to struck-by hazards in that the machine specific energy control procedures were not used.
