29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a) Lehigh Valley Processing and Distribution Center - Employees are rebuilding an Automated Package Processing System, comprised of multiple machines with separate energy sources which have not been controlled. This continuing violation was most recently observed on or about October 18, 2016, when the previously developed hazardous energy control procedures were not applied to the complete machine assembly. The employer's ongoing responsibility is to ensure the adherence to the machine specific procedures to affect the necessary energy control for each system component.
