Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points. (a) The employer did not ensure machine guarding was implemented on the extruder machine. An interlock was missing on a gate that allowed access to moving components that lead billets pass through. On or about November 1, 2016 an employee suffered injuries while clearing debris inside the extruder machine. Tulip Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1059533, citation number 01, item number 04 and was affirmed as a final order on October 28, 2015, with respect to a workplace location at 714 East Keefe Avenue, Milwaukee, WI 53212.
