Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to amputation and struck-by hazards: (a) Great White Pressure Control LLC, Well Ranch BB01-655, WCR 67 and 68, Gill, CO 80624: On and before October 5, 2016 in that employees were exposed to amputation and struck-by hazards caused by the unsafe operation of the pipe wrangler. The employer allowed the pipe wrangler to be operated when the forward movement of the tubing created an amputation hazard between the tubing and a stationary flow pipe located in front of the wrangler trough. The forward movement of the tubing also created a struck-by hazard when the tubing had the potential to strike pressured hydraulic hoses on the blowout preventer. An employee experienced a partial finger amputation when his finger was caught in-between the tubing and a flow pipe.
