29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladders were used to gain access: a) 8023 Buist Ave, Philadelphia, PA - The employer did not ensure that the portable extension ladder side rails extended at least 3 feet above the access surface thus exposing the employees to a fall hazard. This condition was observed on or about October 27, 2016.
