29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): Employees were exposed to finger and hand amputations during the use of hand/foot pedal process to test and position small parts under 4 inches in length with a Accurpress Press Brake, Model 202, and the hazardous energy control procedures did not clearly and specifically outline the techniques to be utilized regarding the following: -specific methods to be used to shut down, isolate, block, and secure the press brake, -specific methods to be used for the placement, removal, and transfer of lockout and/or tag out devices, -specific requirements for testing the press brake to ensure the effectiveness of the lock out, tag out, or other energy control measures.
