29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: (a) The employer did not conduct an annual/periodic inspection of the energy control (lockout/tagout) procedure(s) for equipment, including but not limited to the rubber mill and the vulcanizing rubber press to ensure that procedures and requirements of the standard are being followed, and that employees who implement energy control procedures understand their application. Failure to follow proper lockout/tagout procedure(s) could result in unexpected start-up of equipment or release of energy from equipment, exposing employees to crushing and caught-in hazards.
