29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a. Crown Textile Services Acquisition, LLC - Toledo, Ohio: On or about August 30, 2016, an employee was exposed to an approximate seven and a half foot fall hazard when accessing the top of the G.A. Braun Washer #3 (model 400 NTDP-2; s/n NTDP 40090120) for maintenance activities without the use of fall protection.
