Text For Citation: 01 Item/Group: 001 Hazard:

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of fire and explosion: On or about October 5, 2016, employees depressurizing the well head were exposed to the hazards associated with ignition of hydrocarbon vapors. The employer installed an open top blow down tank with known ignition sources within 100 feet from the tank, and vented the vapors into the atmosphere, where they could come into contact with ignition sources. Ignition sources ranging from 7 feet to 80 feet in proximity to the tank included but were not limited to live electrical conductors, electrical tools, a gasoline generator and running vehicles.
