Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing and struck-by hazards: Employees operated two post Atlas BP9000 hydraulic lifts with a single point lock release lever that had a damaged emergency stopping system exposing employees to a crushing and struck-by hazard. (a) Employees were required to work near and beneath vehicles supported by a defective, hydraulic automotive lift that was known to be in need of repair during an inspection on September 28, 2016, was not removed from service. The inspection showed that an atlas two post lift in the southwest corner of the west repair bay, serial number # 21410143, utilized a screw driver to engage the secondary emergency locking cam, exposing employees to crushing and struck-by hazards. (b) Employees were required to work near and beneath vehicles supported by a defective, hydraulic automotive lift that was known to be in need of repair during an inspection on September 28, 2016, was not removed from service. The inspection showed that an atlas two post lift in the middle along the east wall, serial number # 214101124, utilized a windshield wiper arm to engage the secondary emergency locking cam, exposing employees to crushing and struck-by hazards. (c) Employees were required to work near and beneath vehicles supported by a defective, hydraulic automotive lift that was known to be in need of repair during an inspection on September 28, 2016, was not removed from service. The inspection showed that an atlas two post lift in the northeast corner of the room along the east wall, serial number # 214101140, utilized a screw driver to engage the secondary emergency locking cam, exposing employees to crushing and struck-by hazards.
