29 CFR 1910.178(l)(4)(i): Refresher training, including an evaluation of the effectiveness of that training, was not conducted as required by paragraph (l)(4)(ii) to ensure that the operator has the knowledge and skills needed to operate the powered industrial truck safely: a) On or about September 23, 2017, at the above addressed jobsite - the employer did not ensure employees operating Terex TH844C Model forklift received refresher training to ensure the operators had the knowledge and skills needed to operate the powered industrial truck safely.
