29 CFR 1926.20(b)(2): Frequent and regular inspections of the job site, materials, and equipment were not made by a competent person(s) designated by the employer: a) On or about 09/19/16 at 375 Melville Street, Rochester, New York; the employer did not inspect the scaffold plank to determine its rated load to ensure it was not overloaded during use by employees. Employees were exposed the hazard of a 15 foot fall. b) On or about 09/19/16 at 375 Melville Street, Rochester, New York; employer did not inspect the job site to ensure employees working on a ladder jack scaffold were wearing a safety harness and tied off to a suitable anchorage point.
