Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment: (a) On and before September 2, 2016, employees were exposed to serious injuries, in that the employer had not assessed the hazards in the Electrical Vault Room of the Mill Basement, to determine the use and types of PPE needed when employees are working on or near the 480 Volt Electrical Panels. As a result, an Arc Flash occurred seriously injuring the employee. Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
