29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section. a) Taiho Corporation of America on or about September 7, 2016 at the location of 194 Heritage Dr. Tiffin, Oh, 44883 the employer did not ensure that employees utilized procedures for the control of potentially hazardous energy when an employee was engaged in a setup operation known as a changeover on the MGB-3 line. Employee was exposed to a potential caught by and struck by hazard. The Taiho Corporation of America, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard The control of hazardous energy, which was contained in OSHA Inspection Number 1106347, citation number1, item number 1a and was affirmed as a final order on 03/02/2016, with respect to a workplace located at 194 Heritage Drive Tiffin, OH, 44883.
