29 CFR 1926.251(a)(1): Rigging equipment for material handling was not inspected prior to use on each shift and as necessary during its use to ensure that it was safe: The employer is failing to protect employees from struck-by hazards associated with the improper use of a beam clamp. The most recent occurrence of this was documented on the job site located at 3701 South 10th Street in Omaha, Nebraska where employees utilized a beam clamp designed as an anchorage point for a lifting/hoisting devices. The beam being lifted was unintentionally displaced during the lift and struck an employee.
