29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: For employees working at 4406 West Hillsborough Avenue in Tampa, as observed on or about August 23, 2016. Employees were removing a motor for the propane system when the employee was burned by a flash fire from the releasing gas. The employer did not have a lock out tag out procedure in place to ensure: a) the gas had dispersed enough to prevent an explosive environment. b) the employee was protected from the unintentional closing of the affected breaker in the panel box.
