29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Entire Facility - On or about August 24 through October 26, 2016, operators and maintenance employees performed servicing and maintenance tasks on machinery and equipment such as printing presses and collators. The employer did not develop, document procedures for the control of hazardous energy to be utilized by all employees performing servicing and maintenance tasks. Employees were exposed to amputation, laceration and crushing hazards in the event of unexpected energization or startup. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
