OSH ACT of 1970 Section 5(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushed by hazards while performing septic tank production activities. (a) Septic Tank Department; On or about August 22, 2016 an employee was fatality injured as a result of being crushed by a suspended load. The employees performed work under a suspended septic tank inner core that was not cribbed or blocked.
