Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that; employees were exposed to caught-between, struck-by, and crush hazards: a) Electrified Monorail System; on or about October 18, 2016 and at times prior thereto, the employer exposed employees to caught-between, struck-by and crush hazards, in that; employees were required to enter a mechanized material handling system to perform servicing and maintenance activities, such as but not limited to, equipment upgrades and to reset machinery while the system was operating, and several trolleys had safety bumpers with non-functioning stopping mechanisms.
