29 CFR 1918.65(f)(2): Braking systems or other mechanisms used for braking were not operable or in safe condition. (a) Florida Marine job site - On or about August 23, 2016 an employee was operating a dump truck that went into the Mississippi River. The vehicle braking system were missing parts such as: part of a Brake Booster or Caliper; Diaphragm Cylinder; and Brake Hydraulic sensors.
