29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. On or about August 10, 2016 during the installation of roofing material at the job site located at 1225 Weber Ave Sw, Strasburg, Ohio 44680; the employer did not ensure employees working on the roof of a single family home were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material utilizing air driven nail guns.
