29 CFR 1910.147(d)(5)(ii): Where there was a possibility of re-accumulation of stored energy to a hazardous level, verification of isolation was not continued until the servicing or maintenance was completed, or until the possibility of such accumulation. On or about August 12, 2016, at the Sunoco Logistics Plant in Nederland, Texas, the employer failed to ensure its subcontractor continuously monitored the isolation devices the subcontractor installed in the Seaway header until the servicing work was completed or until the possibility of such accumulation no longer existed.
