29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): On or about July 20, 2016 through August 10, 2016, a Drake Model 5-115 hydraulic press located in department 44 in plant two did not have any machine guarding to prevent operators from accessing the danger zones created by the points of operation between the hydraulic rams and metal parts.
