29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent: (a) Location: PX2 On or about 8/1/16, Employees operating the PX2 machine during 12 hour shifts were exposed to noise at 70% of the action level which is equivalent to 87.4 dBA over an 8 hour time weighted average. Sampling was conducted for 416 minutes and the employer did not administer a hearing conservation program. The action level adjustment for 12 hour shift exposure for a 50% dose is 82.1 dBA TWA. (b) Location: MTI Coating On or about 8/1/16, employees operating the MTI coating machine were exposed to noise at 58% of the action level which is equivalent to 86 dBA over an 8 hour time weighted average. Sampling was conducted for 323 minutes and the employer did not have a hearing conservation program. The action level adjustment for 12 hour shift exposure for a 50% dose is 82.1 dBA TWA.
