29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). On or about July 28, 2016, employees were exposed to a cave in. The employees were installing sewer lines at 6796 Madison, Thompson, Ohio 44086. The employees were working in a trench approximately 7 feet deep without cave in protection.
