Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) 28th Street, McKeesport, PA 15132, Wedge Area; Employees were exposed to being struck by moving towveyor carts transported by the overhead towveyor in that the following deficiencies exist: 1. No training conducted (in accordance with ANSI and the manufacturer). 2. No inspection and maintenance program (in accordance with manufacturer's specifications). 3. No manufacturers approval for removing arelayswitch at the base of the bottom ramp in the wedge area. 4. Backstop Bars/Anti-runaway devices not installed at all exposed ramp areas. Among other methods, a feasible and acceptable method to correct this hazard is to: (i) develop a training program (in accordance with ANSI and the manufacturer), (ii) develop an inspection and maintenance program (in accordance with manufacturer's specifications), (iii) have a representative from the manufacturer review/inspect the area where the relay switch was removed, (iv) install backstop bars/anti-runaway devicesat allexposed ramp areas
